CUHCC accepts most private insurance plans, Minnesota Care, Medical Assistance, and Medicare. If you are unsure whether your insurance is accepted, please call a billing specialist at 612-301-3444.

CUHCC’s Patient Financial Advocates provide assistance with applications for MNsure, Medical Assistance, dental coverage for children, and other programs that might cover your services.
Patients without insurance are welcome at CUHCC
CUHCC serves everyone regardless of their insurance status or ability to pay. CUHCC invites all patients to apply for the discount program.
The clinic provides no-cost mental health care (therapy and psychiatry) to Hennepin County residents who do not have insurance. If you live in another County and have no insurance, the discount program may be available to you for mental health treatment at CUHCC.

- Proof of your household income.
- Names and birthdates of family members.
You must provide the proof of income within 45 days to receive discounted services.

Based on your income and family size, CUHCC will assign you a level from A to F. The cost of a visit depends on the level you are assigned and the type of service you receive. If you pay the full amount at the time of service, you will receive a $5 discount.

*The discount applies to the cost of major dental visits. Patients must also pay 100% of the lab and material costs, if any.

CUHCC will accept any amount that you can pay and bill you for the rest. You can set up a payment plan with the Billing Team by calling 612-301-3444.

The Sliding Fee Discount Program begins the date of your application if you provide your proof of income within 45 days. If you do not provide your proof of income within these 45 days, you will need to pay full price for services. The discount will start the day that you provide the proof of income.

Enrollment in the Sliding Fee Discount Program is good for one year from your application date unless your income status changes. After one year, you will need to reapply for the discount program.

You need to bring at least one of these documents:
- Paystub from work (for last 30 days)
- Most current Tax Return
- Most current bank statement
- Most current retirement benefit statement
- Workers Compensation statement/stub
- Current Social Security statement/stub
- Disability Income statement/stub
- Self-Employed Business Income report or Employer Statement
- Work Verification Letter from an employer
- A Letter of Support from a person you live with
